You are about to miss a compulsory meeting of a course because of illness or other circumstances beyond your control. To register your absence from a compulsory meeting ánd to inform the course coordinator, please fill out this form. Only forms that have been filed prior to the start of the meeting will be dealt with. After you have sent this form, you will receive a confirmation of your notification of absence.

Depending on the nature of the compulsory meeting that you missed, the course coordinator may require that you do a replacement test. It is possible that the course coordinator asks you to submit proof of illness or other circumstances beyond your control. NB, if and when it is needed to submit proof, you will be asked to send this to OWC.AW@uu.nl and this will be handled confidentially by the education coordinator only.
