* If the course that you to list as an extracurriculair course was completed at Leiden University, a screenshot from uSis will suffice.
* Als het vak dat u extracurriculair wilt opnemen behaald is aan de Universiteit Leiden, dan volstaat een screenshot van uSis.
When requesting more than one exemption, it is mandatory to consult your study advisor beforehand.

Wanneer u meer dan een vrijstelling aanvraagt, moet u verplicht van tevoren de studieadviseur contacteren.

There has to be a separate form for every request for an exemption.
So please fill in one form per exemption request.

Voor elk vrijstellingsverzoek moet een apart formulier worden ingediend.
Gelieve dus één vrijstelling per formulier aan te vragen.

 
Describe your request or complaint as clearly as possible and, if necessary, base your decision on the rules (OER).
Describe any demonstrable special circumstances as clearly as possible and add supporting documents if possible.

Beschrijf uw verzoek of klacht zo helder mogelijk en baseer u, indien van toepassing, daarbij op de regels (OER).
Beschrijf tevens eventuele aantoonbare bijzondere omstandigheden zo helder mogelijk en voeg zo mogelijk bewijsstukken toe.

Is de cijferlijst geschreven in een andere taal dan het Nederlands of Engels? Vermeld dat in het formulier en de examencommissie zal laten weten of de cijferlijst wordt geaccepteerd.

Is your transcript written in a different language than Dutch or English? Please note that down in the form, and the Board of Examiners will let you know if the transcript will be accepted.

De documenten die u heeft toegevoegd zullen enkel worden gebruikt om uw verzoek af te handelen en zullen niet worden gedeeld met andere partijen.

The documents provided will only be used in regards to the petition, and will not be shared with other parties.
